An Interview With Dr. KB
Interview : How did you get into this particular branch of medicine? As a child wanted to be a doctor and was very impressed by Dr Hinduja's feature in The Illustrated Weekly of India. As I joined MD in Gynecology was fascinated by this subject and hence my specialisation.
What motivates you in this field? The happiness, good wishes of all successful couples..and the frustrations of all those who have not made it. Yet..
What is good patient care? Understanding the pain, clear communication, a good IVF lab and excellent clinical services.
How do you address a patients emotional needs? A patient initial consultation where all issues are addressed. The team then takes over to comfort them through the journey. I am always there as I do all procedures myself including their scans.
Who is an ideal patient? And an ideal infertility doctor? An ideal patient has made up her mind. She understands the process and trusts the doctor. She is hoping for the best, but is prepared for the worst. An ideal infertility doctor is well trained, has an excellent staff, has good communication skills and is humane.
Can you describe the most challenging case you have handled so far? One case involved an American lady with repeated IVF failures elsewhere. We had tried twice and I was not willing to try anymore as her egg quality was not good nor the endometrium. She still wanted to try and went ahead with 3 more cycles elsewhere. After being unsuccesful, she came back with her sister to try a donor egg IVF cycle. I suggested we go ahead with surrogacy. She was still keen to conceive in her own womb, understandably. That failed. By now, she had come to terms with her condition. She was open to surrogacy. She did 2 cycle with me. Both were successful and now she is a happy mother of a boy and and girl. In this long tiresome journey I must also give credit to her husband who stood by her always. Another involved a middle class couple from New Delhi. They had undergone two IVF cycles elsewhere. I did an ultrasound and found what is called a hydrosalpinx, which is water in the fallopian tubes. This can be harmful to the growing embryos in the uterus and reduces success by half. I went ahead sand did a laparoscopic procedure where we disconnected this damaged tube with the uterus. We tried an IVF cycle which failed. Fortunately, we had frozen enough embryos. We thawed the embryos and transferred three. She became pregnant. Today the couple are proud parents of a beautiful daughter. The third which I want to highlight was not a technology driven case, but I remember the relief on the couples face when I told them they did not need any expensive treatment. This couple were unable to consummate their marriage for the past 5 years. They had been to all kinds os specialists and tried everything. They were now desperate to have a child. They were told this could never happen till they consummate or IVF was the only option. They were mortified of IVF. I assured them that this was not necesssary and explained a simple technique called Artificial Insemination which they could practise at home. The couple were immensely relieved. They came back 4 months later with beaming faces and a positive pregnacy test.
How would you explain IVF to a layman? And what all is involved in it? IVF means In Vitro Fertilization, meaning fertilization outside the body as opposed to In Vivo Fertilization which is fertilization inside the body, which happens normally. In this a woman is given hormonal injections for 10 days from the second day to create many mature eggs as opposed to a single egg in a menstrual cycle.

What is Infertility
An Overview In simple words infertility is defined as the inability to conceive. And this is the reason, it requires thorough diagnosis of both the partners. Once, this malady is diagnosed, there is every possibility that it could be treated successfully.
However, there are other factors as well like age, general health etc., that may affect the success of treatment.
At the same time, with technological advancements in the field of surgery and/or assisted reproductive technologies (ART), such as in vitro fertilization (IVF), majority of infertility cases can be treated successfully.
Infertility in women The common cause of Infertility in women is disruption of ovulation symphony by disturbances in menstrual cycle. Instead of a regular cycle, the women suffering from infertility undergo irregular cycles. In the wake of this, menstrual history is considered to be the first instance of infertility, which one should seriously take care of and immediately consult the doctor.
The Menstrual Pattern is the simplest screening test to guide medical investigations.
The regular menstrual period: In this category, the periods are regular from month to month, beginning like clockwork every twenty-five days or every thirty-five days. The consistently irregular menstrual cycle, where one month you begin menstruating after twenty-five days, the next month after thirty-four, and the next in thirty, may indicate that you have fertility problem.
Irregular menstrual periods or Amenorrhea for six or more months: Irregular periods aren't unusual, they affect about 30% of women in their reproductive years. The woman's menstrual periods occur infrequently and at unpredictable intervals. Even in some of the cases, the menstruation at a period of time, may suddenly stop altogether.
Nonexistence of the menstrual period: If a women does not have menstruate even after the age of 16 years, which is considered to be the maximum limit to start menstruating, it is considered to be a serious condition. The cause of such conditions may include genetic abnormalities, congenitally deformed reproductive organs, delayed puberty, or a pituitary malfunction.
What is Ovulation?
Ovulation is the process in the menstrual cycle by which a mature ovarian follicle ruptures and discharges an ovum (also known as an oocyte, female gamete, or casually, an egg) that participates in reproduction. The entire process of ovulation is controlled by the hypothalamus of the brain and through the release of hormones secreted in the anterior lobe of the pituitary gland, Luteinizing Hormone (LH) and Follicle-Stimulating Hormone (FSH). A spike in the amount of FSH triggers ovulation and LH is released from the pituitary gland.
Causes of Infertility
There are various conditions that may cause infertility in the women. These conditions include the following:
Endometriosis: It is a common medical condition in which the endometrial tissue, the tissue that normally lines the uterus, grows outside the uterus and attaches to other organs in the abdominal cavity such as the ovaries and fallopian tubes. Endometriosis is a progressive disease that tends to get worse over time and can reoccur after treatment.
The most common symptom of endometriosis is pelvic pain. The pain often correlates to the menstrual cycle, but a woman with endometriosis may also experience pain that doesn’t correlate to her cycle. The other symptoms include abnormal menstrual bleeding and pain during or after sexual intercourse. Endometriosis can also cause scar tissue and adhesions to develop, that can distort a woman’s internal anatomy.
A laparoscopy, an outpatient surgical procedure, is necessary to confirm a diagnosis of endometriosis after a medical history review and pelvic exam. After the diagnosis, our physician will classify your condition as stage 1 (minimal), stage 2 (mild), stage 3 (moderate) or stage 4 (extensive). Based on the stage of the Endometriosis, the physician may decide the best treatment plan.
Polycystic ovary syndrome (PCOS) Polycystic ovary syndrome (PCOS) is the most common endocrine abnormality that affects six to ten percent of women, who are in the reproductive age group. PCOS is a leading cause of infertility and also affects the woman's cardiovascular health. The causes of PCOS are unknown. Researchers believe that the causes of PCOS may be insulin resistance and an imbalance of female hormones (estrogens and progestins, those important for the menstrual cycle) contribute to the development of this condition.
The symptoms of the disorder include acne, difficulty becoming pregnant (infertility), irregular menstrual periods, obesity, skin discolorations and unwanted hair growth or loss.
New medical researches have revealed different methods of treatment including insulin-reducing ovulation medication (clomiphene, glucophage, metformin), dietary changes (low glycemic diet) and surgery (ovarian drilling). Women who undergo treatment for PCOS, but are still unable to conceive naturally, often turn to assisted reproductive technologies such as IVF.
Tubal diseases One of the more common causes of infertility is - blocked or damaged fallopian tubes. The fallopian tubes are necessary for picking up the egg, released from the ovary. It is in the inside of the fallopian tube that natural fertilization occurs. Scar tissue resulting from endometriosis or abdominal or gynecological surgery, such as bowel surgery, cesarean section or a ruptured appendix, can block an egg from entering or traveling down the fallopian tube to meet the sperm, preventing fertilization.
Such condition may cause infections including chlamydia, can damage the cilia, the tiny hairs lining the fallopian tubes that help transport the egg, often preventing the sperm and egg from meeting. One result of damaged cilia is an ectopic pregnancy, which occurs when an egg is fertilized but, due to the damaged cilia, it is unable to travel to the uterus, growing instead in the wall of the fallopian tube. This condition can result in rupture, internal bleeding and further tubal damage.
The physician reviews the complete history and does a complete pelvic exam to diagnose the tubal disorder. For the confirmatory diagnosis, additional tests such as hysterosalpingogram and laparoscopy are required and the exact course of treatment is decided on the basis of the condition.
Q. If I conceive, will my baby be normal? A. Studies have shown that there is no increased risk of abnormality in IVF conceived babies compared to those conceived naturally.
Q. What are the precautions I should take after the IVF procedure? A. The procedure demands no special precautions, but avoid strenous activity. We advise the patients to be mentally & physically relaxed as much as possible. You can return to work if you wish, but fist prefer to have a few days rest.
Q. What if IVF fails the first time? How many times will I have to undergo the same procedure? A. You can go through IVF as many times as you wish, but we advise up to five cycles at the most.
Q. Is IVF the last option? A. IVF is the option, which has highest success rate and is also the most expensive. But, it is not "end of the road". Many women have conceived naturally or with intrauterine insemination, even after IVF. However, for those with blocked tubes & very poor sperm counts, it is only option.
Q. Is IVF very expensive? A. IVF is not as expensive as percieved in general. Generally, the cost of IVF cycle depends upon the dose of drugs that would be needed for ovarian stimulation. It is only as expensive as perhaps gall stone removal or removal of uterus surgery.
Q. Do we need to get admitted in the IVF process? A. A patient undergoing IVF does not require admission. However, one should visit the center 3-5 times during monitoring cycle. On the day of egg collection, the patient would need to fast for 6 hours & come to clinic (the procedure takes about half an hour). Patients can go home after the effect of anaesthesia weans off which takes about 2-3 hours. The next scheduled visit is after 2-3 days for the embryo transfer, which again takes about half an hour and patients are free to go home after resting for one hour.
Myths And Facts
Myth : Infertility is a women's problem. Fact : This is untrue. It surprises most people to learn that infertility is a female problem in 35% of the cases, a male problem in 35% of the cases, a combined problem of the couple in 20% of cases, and unexplained in 10% of cases. It is essential that both the man and the woman be evaluated during an infertility work-up.
Myth : If you adopt a baby you'll get pregnant! Fact: This is one of the most painful myths for couples to hear. First it suggests that adoption is only a means to an end, not an happy and successful end in itself. Second, it is simply not true. Studies reveal that the rate for achieving pregnancy after adopting is the same as for those who do not adopt.
Myth : I shouldn't take a month off from infertility treatment for any reason... I just know that this next month will be THE one. Fact : It is important periodically to reassess your treatment and your parenting goal. Continuity in treatment is important, but sometimes a break can provide needed rest and renewal for the next steps.
Myth : I'll be labeled a 'trouble maker' if I ask too many questions. Fact : The physician/patient team is important. You need to be informed about what treatments are available. What is right for one couple may not be right for another, either physically, financially, or emotionally. Don't be afraid to ask questions of your doctor. A second opinion can be helpful. If needed, discuss this option with your physician. .
Myth : If I have taken oral contraceptive pills before, I will find it difficult to get pregnant later. Fact : The use of oral contraceptive pills in the past to avoid pregnancy does not reduce the chance of getting pregnant in the future when the couple desires, as is commonly believed.
Myth : IVF is my last option, if I don’t get pregnant in this cycle I will never get pregnant. Fact:IVF is one of the effective ways of getting pregnant, however it is not the last option. Many couples get pregnant after an IVF attempt even spontaneously.
Myth : IVF is very expensive. We cannot afford it. Fact : Though IVF is expensive, it is not as expensive as it is imagined. One cycle costs about 1.5 lakh inclusive of all drugs. This is almost equal to the gall bladder surgery or caesarean section in private hospitals. .
Myth : Undergoing a Laproscopy surgery will further worsen my infertility problems . Fact : Laproscopy in infertility is a skilled procedure. It is very useful to diagnose and treat female infertility problems. There is spontaneous pregnancy rates upto 60 % in some cases..
Myth : I must confine myself to a certain fixed diet plan to get pregnant and stay pregnant. Fact : There is too much emphasis given to dietary do’s and dont’s. People from all over the world, right from Africa, Mongolia, Europe and Asia are getting pregnant! They all have varied dietary patterns. It is important to have a balanced nutritious diet. .
Myth : Bed rest is essential to get pregnant after IVF . Fact: : A woman should not undergo excessive physical or mental stress after the procedure. Strict bed rest is not associated with increased pregnancy rates.

Extensive andrology work is done at our centre as we believe in treating the couple as a whole instead of just managing the women and referring the males in case they have fertility. We perform Intra Uterine Inseminations whenever possible and do not direct all our patients towards IVF/ICSI unless their tests leave them with no other option. Within the same lab, in a separate room, lies the cropreservation facility. This is one of the most indispensable areas of the clinic. It is where vitrification of sperms, oocytes as well as embryos is done for future use if necessary. In fact several patients both indian and international keep their embryos frozen even while they have conceived a child from IVF. Then they opt for Frozen embryo transfer in order to conceive another child with good results.
We have an very good, quiet, peaceful and well attended post operative area for patients undergoing IVF procedures or general laparosopic/hysteroscopic procedures to enhance fertility. Our surgical operating theatre is separate from the IVF operating room and lab so as to maintain the highest standards to cleanliness. Embryos are extremely sensitive to any changes in local incubator environment, any synthetic additive in their lab enviroment (such as perfumes etc) are embryotoxic.Hence the general gynecology OT are kept separate. In the general OT we perform laparoscopy and hysteroscopy and other procedures with our state of the art equipments f and experience in handling over 5000 IVF cases.
Apart from the OTs, we have consultation chambers, and a separate injection room where the cold chain is maintained between 2-8 degree celcius as recommended. Here in the privacy of the chambers the patients freely talk, share their issues with the consultants and receive experienced advice. They recieve the daily injections in the injection room by specially trained nursing staff.
Perhaps one of the most important facts about the centre is the availablility of the Senior most consultant and director throughout your cycle instead of just meeting you on the first day and the embryo transfer day. Here the consultants do your ultrasounds themselves, discuss your treatment with you themselves, answer each and every query themselves. The Director Dr Banerjee is usually available throughtout the month, and supervises all treatments and procedures. She herself does all egg pick ups and embryo transfers.
